Health Net*, on behalf of CalViva Health, recently made a significant investment to offer Cozeva at no cost to our participating providers. Cozeva is a reporting and analytics platform that will allow you to better monitor and take action on performance gaps for quality and risk measures.

Advantages of using Cozeva:

• Receive more timely data on Quality/Risk performance and track estimated and potential incentive calculations in real time. Trend your scores over time and see how they compare to other providers in our local network.
• Review open care gaps and chronic conditions for your CalViva Health patients. Use batching and bulk printing of face sheets to facilitate pre-visit planning.
• Close care gaps instantly by submitting attestations for measure compliance or exclusion and uploading proof-of-service documentation.
• Get direct-to-provider incentives for select programs paid more often (quarterly vs. semiannually) with secure E-payments through PayPal Hyperwallet.
• Current Cozeva users can link their existing account to the health plan’s platform and obtain a unified view of patient conditions and gaps through Cozeva Client Bridge.
• Live support chat for technical issues (5 a.m.–8 p.m.). Use HIPAA-secure messaging to connect with the health plan teams and other Cozeva users.

Two ways to register:

Create Cozeva accounts for all providers and staff involved in efforts to track, address and close patient care gaps – there is no limit to your office’s number of Cozeva users.

Provider Relations representatives will be contacting eligible primary care physician (PCP) offices to offer Cozeva and begin registering their users.
