20-113 Complete the Special Needs Plan Model of Care Training

This CMS-required training must be done each year

The Centers for Medicare & Medicaid Services (CMS) model of care (MOC) training required for Health Net* Medicare Advantage (MA) providers is available online. The training is designed to help you better understand Health Net’s clinical practices and general approach to the delivery of care for Special Needs Plan (SNP) members.

Access the training in three easy steps to comply

SNP participating providers must complete the SNP MOC within 90 days of contracting, hiring or becoming a delegated entity, and yearly thereafter (by December 31 of the year). Evidence of annual training must be maintained and on record at your office in the event of a CMS audit.

To access training:

1.     Go to provider.healthnet.com.

2.     Select Working with Health Net > Regulatory > Medicare Information > Model of Care Initial and Annual Training.

3.     Select Model of Care Annual Training (pdf).
