Employer Group PPO Access Card Travel Program

The following information does not apply to individual family plans (IFP).

The Health Net PPO Access Card Travel Program is available to all PPO members, providing access to a nationwide network of participating PPO providers at in-network coverage levels while traveling or temporarily residing outside their state of residence (such as while attending college). The PPO Access Card Travel Program is not available for members who travel outside their state of residence in order to receive medical care or who have established permanent residency in another state after enrollment.

Provider Network

Health Net has entered into an agreement with First Health to provide a nationwide network of participating providers for Health Net PPO members. Health Net PPO members may access the First Health network of providers when traveling outside their state of residence. The First Health logo appearing on a California member's identification card applies to the PPO Access Card Travel Program only.

Medical Management

The Health Net Medical Management Department performs medical management for Health Net PPO members who reside in California and access care while traveling outside California.

First Health performs medical management functions for Health Net PPO members who reside outside of California and access care while traveling in California. The First Health Medical Management Department telephone number is listed on the commercial and Medicare prior authorization requirements, and on the back of the member's identification card.

Claims

Health Net PPO participating providers must continue to submit claims to the Health Net PPO Claims Department for processing.
