Part D Enrollment

To be eligible to elect Cal MediConnect, a member must be entitled to or enrolled in Medicare Part A, enrolled in Part B and eligible to enroll in a Part D plan as of the effective date of coverage. Members must also permanently reside in the service area.

A beneficiary is not eligible for Part D when they are incarcerated or live abroad.

Members with a diagnosis of end-stage renal disease (ESRD) at the time of enrollment are not eligible to enroll in Cal MediConnect plans offered in Los Angeles and San Diego counties unless they are already enrolled in a separate line of business operated by Health Net. Members enrolled in Cal MediConnect who are subsequently diagnosed with ESRD may choose to disenroll from Cal MediConnect or may choose to stay enrolled.
